New Patient Registration

To register as a patient- providing you live in the practice area (see map below) you need to supply your medical card or complete a new patient registration form- available at reception. We ask for proof of address via a utility bill which is also asked when changing address.

We advise all new patients to make an appointment with the practice nurse for a new patient check, to obtain details of your medical history and current medication as receiving this information from previous surgeries may take time.

  • The freedom of information act 2000 obliges the practice to produce a publication scheme as a guide to the ‘classes’ of information the practice intends to routinely make available.
  • If you change your contact details please let the practice know as soon as possible.

Practice Area

Named Accountable GP for all Permanent Registered Patients

All practices are required to provide patients with a named Accountable GP. The aim is to ensure that there is a GP in the practice who can offer overall responsibility for the care and support that our surgery provides for you. They may also work with other health care professionals to ensure that you have a co-ordinated package of care.

All patients have been allocated a named Accountable GP at the practice. Whilst this doctor has been designated as having overall responsibility for care and support that our surgery provides for you, this does not prevent you from seeing any other doctor in the practice as you currently do.
